By filling out this questionnaire, you are providing your personal information (name, surname, phone number, etc.) to AzerMDS. This information will be used by the AzerMDS Board of Directors only to monitor your activity and membership status. Your personal information will only be seen by the AzerMDS Board of Directors and will not be transferred to third parties under any circumstances. Your stored information will be deleted by us immediately upon termination of your activity within AzerMDS.
